Sub: Revised BAI Age Certificate form for Players
We are enclosing herewith the Revised BAI Age Certificate form for Players which has been implemented by BAI.
You may please note that henceforth BAI ID will be issued only after submission of the new age certificate form and the undertaking in Rs. 50/- Stamp Paper (Under-13, 15, 17 & 19 Age Group Players) duly notarized. A copy of the new form is given in our website also.
It has been noticed that many District Secretaries are not giving the date while signing the forms. Please ensure that all the columns given in the form are properly filled in.
In this connection, please refer to our Circular dated 26th August, 2015 wherein we have mentioned that without the undertaking in Rs. 50/- Stamp Paper duly notarized for Under-13, 15, 17 & 19 Age Group Players, registration of players will not be made / renewed for the forthcoming year.
