Sub:     Affidavit on Rs. 50/- in Stamp Paper to be attached with the Registration Form

 

Dear All,

 

It is being noted that Players / Parents are not following proper filling up of the Affidavits of the Players concerned in 50 Rupees Stamp Paper which is creating Hazards at our end.

 

We give example as under:

 

1)        In the 1st Para in the third line it is written “Son / Daughter............” which means if it is daughter the word son has to be striked out.

 

2)        In Sl. No. 2 in the first line it is clearly written that “the above date of birth of our Child.......................... is true” it means the name has to be put here.  In many cases, date of birth is being given.

 

3)        In Sl. No. 2 Line No. 4 & 5.... Only concerned District name and West Bengal Badminton Association are to be written.  For example now there are Five (5) Districts under Kolkata “North Calcutta Badminton Association, West Calcutta Badminton Association, Central Calcutta Badminton Association, South Calcutta Badminton Association & East Calcutta Badminton Association”.  Hence, the word District will not come in this case.

 

Please do follow the above strictly while submitting the Registration Forms.

 

In this context, we would also like to mention that whenever a District is issuing a NOC for a Player for another District, they should send all the relevant papers such as Affidavits, Medical Forms etc. along with the NOC.  The District concerned issuing the NOC should immediately give in the Portal of their District that “NOC” of the player has been issued. 

 

It may also be noted that the concerned District where the player concerned is going to register his name, should upload all the papers along with the new Form in their respective Portal.
