Sub: Undertaking of Parents on Rs. 50/- Stamp Paper for Under-13/15/17 & 19 Age Group Players.
Dear Sir,
Please refer to our letter dated 31st December, 2014 vide which we had requested you to send the
Undertaking for Under-13/15/17 & 19 Age Group Players on Stamp Paper (Rs. 50/-) duly notarized
as to the correctness of the age as per the format enclosed therewith and given in our website. We
have also not received revised BAI Age Certificates of many players duly signed by the Parents, as
circulated earlier.
You may please note that BAI has taken a decision that without the Undertaking, no ID will be issued. We give below the excerpts of the guidelines issued by BAI:
QUOTE” 1. ALLOTMENT OF PLAYER I.D – BORN AFTER 1ST JANUARY 2014.
The following system will effect strictly from 1st January 2014 onwards.
The Players should produce the certificate be dated of the same year of birth from the Registrar of Birth / Panchayat / Municipal Administration with BAI proforma and affidavit on Rs:50/- court stamp confirming the DOB, place and venue of Birth, Name of Hospital/ Nursing home etc.
2. ALLOTMENT OF PLAYER I.D – BORN BEFORE 31ST DECEMBER 2013.
a. BAI I.D form – to be signed by the District, State Associations with Thumb & signature of candidate. (After Verification of DOB).
b. Date of birth certificate from school / college / Panchayat / Municipality/ Registrar
of Birth.
c. Affidavit by parents on Rs:50/- court stamp, confirming the DOB, place and venue of birth , name of the Hospital / Nursing home etc.
3. REGISTERED PLAYERS OF BAI.
Those players already allotted BAI-ID and not submitted parent affidavit are requested to submit the parent affidavit on Rs:50/- court stamp, confirming the DOB, place and venue of birth , name of the Hospital / Nursing home etc. Minimum 30 days required to issue BAI ID after receipt of I.D application through respective member Associations” UNQUOTE
It has been noticed that many Districts have still not forwarded Undertaking of the players, as required as also many are being sent incorrectly. Please treat the matter as Urgent and forward the Undertakings as also revised BAI Age Certificates duly signed by the Parents (who have not sent) in proper format of Registered Players of your District at the earliest.
